Roughly 45 minutes from cold to about 65°C, so switch it on when you arrive rather than when you want the shower. Once it is up to temperature the insulation holds it a good while, and the auto re-ignition tops it back up without you touching anything.
60°C for most days — plenty for a shower and it stretches both the bottle and the tankful further. Choose 70°C when several people shower back to back, since hotter water mixed with cold goes further. Mind the kids at the tap either way.
Anywhere you have a bottle and 12V. The ignition and controls run off the house battery, drawing 0.16A to light and 0.12A while heating, which is nothing. No mains needed, so it suits people who camp away from powered sites most of the time.
