Does this include the sway controller itself?

No, this is the mount that puts a friction sway controller on your A-frame without drilling. If you do not already have the controller, you need both parts. The kit exists because putting holes in an A-frame is a decision plenty of owners would rather not make.

Why avoid drilling the A-frame at all?

Because the A-frame is structure, and holes in the wrong spot put a stress riser exactly where you do not want one. Some van builders take a dim view of it too. Clamping around the rail keeps the metal intact and lets you move the mount later.

Is friction sway control enough on its own?

For a light camper on sealed roads it takes the edge off. On anything heavier, friction damping is a partner to a weight distribution system, not a substitute. It slows a sway that has already started, where correct weight distribution and loading stop it from starting.
