How do I know it is the thermostat and not the cooling unit?

The tell is regulation, not cooling. If the fridge gets cold but ignores the dial, over-freezes everything, or the gas burner never cycles off, that is thermostat behaviour. If it never gets cold on any mode, you are looking at the cooling unit or the burner instead.

Do I need a separate part for the 12V side?

Yes. This one regulates the gas and 240V circuits only; 12V on these fridges runs through its own control. That catches people out who replace the thermostat, find gas and mains behaving, then assume the new part is faulty when 12V still runs flat out.

Is this a job I can do myself?

For most people, yes. It is a sensor and capillary swap, so the work is pulling the control panel, transferring the knob and mounting hardware from the old unit, and clipping the new sensor back onto the fin at the same depth. Isolate gas and mains first.
