Do I need to cut a recess for it?

Yes. A flush lift ring drops into a cut-out so the ring lies below the surface when it is not in use. That means a hole saw or a router and a careful template, not just a few screws. Measure twice, because a flush fitting shows a sloppy cut.

Where is it worth using?

Anywhere a raised handle would be a hazard or a nuisance: floor hatches, under-bunk lids, fish box covers, a ute canopy floor. If the lid is on a vertical face or somewhere nobody walks, a surface-mounted handle is easier to fit and easier to grab.

Is it hard to use with wet or cold hands?

That is the trade-off. A recessed ring gives you less to grab than a raised handle, and on a wet deck with numb fingers it can be fiddly. If the hatch is heavy or you open it often, think about whether flush is worth the daily fumble.
