How do I know my shackle will fit through it?

Measure your shackle pin first. The one figure given is 38mm, and on a cast pad eye that describes the plate rather than the hole, so the opening is smaller than the number suggests. Diamond pattern puts the bolt holes on the diagonal, which spreads the pull over a wider patch of deck.

Can I bolt this straight to a fibreglass deck?

Not without something behind it. A pad eye takes an angled pull, and bolts through thin laminate will dish the deck or pull the core apart. Use a backing plate or at minimum oversized washers, and bed the pad on sealant so water stays out. Spread the load and the fitting is no longer the weak point.

Is a cast pad eye as strong as a forged one?

Casting lets you have the diamond profile and a smooth radius where a line bears, which is kinder on rope. The trade-off is that cast stainless has no grain direction working for it, so it suits tie-downs and accessory mounts rather than sustained rigging tension.
