Vibration. A turnbuckle under a steady load will slowly unwind itself as the boat works, and lifelines lose tension before you notice. Wind the nuts down hard against the body once you have set the length, and they jam the threads so the barrel cannot turn.
Keep thread engaged in the barrel at both ends, as a rule at least as much thread buried as the diameter of the stud. Wound out near the end of its travel a turnbuckle looks tensioned and is holding on by a few turns. Get a longer one instead.
No, that is the classic way to kill a stainless turnbuckle. Tape traps salty water against the threads with no oxygen, and crevice corrosion works away underneath where you cannot see it. Use split tubing or leather boots that let it breathe and drain, and rinse under them.
