Should I run it on gas or 240V?

Gas when you want hot water quickly, 240V when you are parked up and in no hurry. It pulls 8.7MJ/hour on gas, which is modest against a full bottle, and on a powered site you can leave the element to do the work instead.

What is the 12V side for if it runs on gas and 240V?

Ignition and control. The 12V feed fires the spark and runs the switch inside the van, so you light it from the kitchen instead of crouching at the external hatch. No 12V means no gas operation, which catches people out when the house battery goes flat.

Is 20.3 litres enough for a family?

For a couple, comfortably. With kids you will be managing turns rather than running the tap freely — 20.3L empties faster than people expect once someone washes their hair. Recovery at 40L/hr at a 45°C rise means the wait between showers is short if you stagger them.
