As high as you can without it becoming a handle or a hazard. Height is what gets you range on VHF, so the top of a cabin, a targa or a rail beats the gunwale. Keep it clear of where people grab and where the cover goes on.
Because it sits in the worst spot on the boat. An antenna mount is high, permanently exposed and takes the full hit of salt spray, sun and wind load. A marine-grade stainless base will still be solid when a plastic or plated mount has gone chalky and loose.
Backing plates and sealant, plus a way to run the coax. Bolting a mount through thin fibreglass or an alloy gunwale without spreading the load will crack or dish it over time. Bed the base in a marine sealant so water never tracks down the bolts.
