What does the 23/42mm in the name mean?

It is the roof thickness this version clamps onto without cutting or adapters. Measure at the edge of the opening, through the full sandwich of skin, foam and inner lining. If your tape reads beyond that band, you want the thicker-roof version instead, not a packer to make it work.

Do I have to cut a bigger hole in the roof?

If you are replacing a smaller hatch, yes, and that is not a weekend job to rush. A bigger opening in the roof can cross a roof bow, and cutting through it without adding support is how a roof starts to sag. Have a van repairer look at the framing before the saw comes out.

Is the bigger opening worth the extra heat it lets in?

More glazing means more airflow and a much brighter interior, and it also means more heat in and more heat out. The blind is what makes it liveable, so use it: shut in the afternoon, open in the evening. If you camp mostly in the tropics, that habit matters more than the size.
