Do I actually need a breakaway unit?

If your van is over 2000kg you do, by law, so this is a compliance item rather than an accessory. Under that weight it is optional and still sensible on long tows. Either way it is the device that stops an unhitched van from running on its own down a hill.

It runs off my house battery, so what if I am on lithium?

This one is specified for lead-acid house batteries above 50Ah, so talk to your installer before wiring it to a lithium system. The payoff for drawing from the house battery is that there is no separate breakaway battery quietly going flat in a box on the A-frame between trips.

What does the Bluetooth part do day to day?

It lets you confirm from your phone that the unit is armed and healthy before you pull out, instead of trusting a test light you cannot see from the driver seat. A breakaway system is the one bit of safety gear you hope never works, so knowing it is alive matters.
