Flush mount or in-line — which should I fit?

Flush mount if you want it always connected and never forgotten in a hose bag. The trade-off is a hole cut in the van wall and about 80 mm of clear space behind it, so you need a bay or a void with no wiring or plumbing in the way.

Why knock the pressure back at all?

Van plumbing is thin-wall pipe and push-fit joints, and park mains can sit well above what it was built for. Knocking the feed back protects the joints you cannot see, behind cabinetry and under the floor. The damage from a let-go fitting is always worse than the pressure you gave up.

Will I notice weaker flow in the shower?

A regulator caps pressure, it does not throttle flow, so a shower fed from a decent supply feels much the same. Where people notice a drop is on a long, skinny hose or a park with weak mains to begin with. Fix the hose before you blame the regulator.
